Step 1: Review Your Plan Options

Click the button below to view available health plans and monthly premium amounts.

NOTE: This site is for viewing options only—do not enroll in a plan yet.

Step 2: Do Not Enroll on Your Own

Do NOT enroll directly through the site. Doing so will prevent MEDSURETY from assisting with your policy setup or processing your premium payments. A MEDSURETY Policy Placement Advisor will guide you through enrollment to ensure everything is completed correctly.

Step 3: Schedule Your Enrollment Meeting

When you’re ready to move forward, click on your state to schedule a meeting with a Policy Placement Advisor.

What to Have Ready for Your Appointment:
  • The plan name and premium amount you are interested in
  • Date of Birth and Social Security Number for You, Spouse, and Dependent(s)

Select your state.
If your state is not listed, contact MEDSURETY.

Step 4: Meet with a Policy Placement Advisor

During your scheduled meeting, the Policy Placement Advisor will guide you through the enrollment process to ensure everything is completed correctly. Specifically, they will:

  • Help you review and select your medical plan
  • Complete the ICHRA Enrollment Form

Step 5: Cancel Your Previous Coverage

After your meeting, your Policy Placement Advisor will email you the carrier cancellation form. It is your responsibility to complete and submit this form to your previous insurance carrier to officially cancel your old policy. This ensures you won’t be double-billed or have overlapping coverage.
